Immunizations are required to be up to date and current for all grade levels including Pre-K and Early Childhood.

Sports physicals are required prior to participation in the first day of practice in all sports. |
School Physicals are Mandatory for Students Entering: Pre-K Early Childhood Kindergarten Grades 6th & 9th Dental exams are Mandatory for Students Entering: Kindergarten Grades 2 & 6 |
All Health Records must be on file with the School Nurse by the first student attendance day. |
